That morning my front rack bail caught a car door in Seattle

Last Tuesday I was doing my usual 4 mile ride down Pine St when my loose bungee cord snagged the mirror of a parked Prius. The guy opened his door right as I passed so I had to swerve into the bus lane, nearly ate it on a manhole cover. I pulled over, checked the rack, and realized the hook had been slipping for a week. Now I use two short straps with carabiners instead of one long cord, cost me $8 at a hardware store. Has anyone else had a cargo setup fail at the worst possible moment?
