Did the building plans get approved by a city inspector or did someone just sign off on it without checking?

No way, I gotta push back here. If the city signed off on it then that's official approval, end of story - they literally have inspectors for that exact reason. People act like every permit is shady but most of the time it's just standard paperwork, not some big conspiracy.
