Can we talk about how people review sleeping bag temperature ratings?

I keep seeing folks complain their 20-degree bag was cold at 30 degrees, but they were using it on a basic foam pad. The rating assumes proper insulation underneath, something I learned the hard way on a cold night in the Adirondacks. What's the most misunderstood spec you've run into?
